Always best to have an in person examination to determine implant type, shape, and size. Regarding placement, either above or below the muscle has great results in an athlete. I totally understand how you can feel discouraged. Due to your unique anatomy, your nipples/areolas and breast tissue are a bit wide on your chest wall. The challenge here is balancing fullness at your cleavage vs centralizing the implant under your nipple/areola complex. Based on your photos, your implants have migrated down and there is now a "double bubble" deformity. You will need your implant pocket revised and a lift, especially on the side where the nipple/areola is lower. Definitely wait until you have completed breast feeding your little one.

As of now, there are no techniques to create new fat cells. If you do not have your own fat to transfer to your breasts, the only other option is cadaveric fat matrix which recruits your own fat to the area where its injected. This can be pricey and may not be very effective to get you the volume you desire. A breast augmentation with an implant may be your only option.

So good news, you do not have tubular breasts, which are a bit more challenging to correct. You are a good candidate for a breast augmentation with placement of the implants below the muscle. The other thing to mention is that your nipples and areola are a bit wide on your chest and their placement will not change after an augmentation, so your cleavage after augmentation may be wide as well. Whether or not the pectoralis muscle can be repaired after removal of implants and the capsules surrounding them is dependent on where the muscle sits after your original surgery. The assumption here is that your implants are below the muscle. Yes, liposuction will reduce the size of your larger breast to match the volume of the other. That being said, will you achieve "decent results"? That really depends on what you are hoping for. If you are not concerned about the sag of the breasts due to the excess skin, then your liposuction can make your breasts look more symmetric in volume, but your the nipple and areola of the larger breast will still sit lower than the smaller side. This is a helpful question for many post-op patients. It is not uncommon to have pain and bruising that evolves as you are healing. Especially after stopping pain medication, you might begin to feel a bit more than when you were on it. You definitely always want to contact the office of your Plastic Surgeon, even if she/he is not in town as they can help to guide you to your doctor's general plan for such events.

You're right to question how your medical and surgical history may affect your outcome after a tummy tuck or mommy makeover. The scars from the surgery you have undergone can be significant and I am sorry for that. I recommend waiting at least 12 months from the date of your ileostomy reversal before undergoing a tummy tuck. There is a lot of scarring in the area and there may even be a hernia to repair so its best to let the scar tissue and inflammation completely settle before going into the area. There are a few differing viewpoints regarding this issue, so great question! When it comes to fillers under the eye, you are right to be cautious about who is doing the injecting. Best to go to an Oculoplastic or Plastic Surgeon as she has the best understand of the anatomy of the eye (either one will be great).
