Thank you for your question! Reviewing your photos it appears the best course of action is a breast lift in addition to implants. Due to the degree of that your breasts sag the best cosmetic result would be a vertical (lollipop incision) breast lift. This will bring your nipples back up to their optimal position and will enhance the fullness you will achieve in addition to the implants.

In my experience very few patients are good candidates for liposuction alone as a means of breast reduction. Although younger patients may not have a great deal of ptosis (sagging), they may have very dense breasts that are not amenable to liposuction. Older patients may have a great deal of ptosis; therefore, liposuctioning the breasts will only make this worse. Most times you have to remove the excess skin with the breast tissue giving the patient a lift with the reduction.

You appear to be an excellent candidate for a breast reduction. Suture selection is a matter of surgeon preference. I prefer to use absorbable monofilament (single stranded) sutures as opposed to braided sutures. Monofilament sutures are less prone to elicit inflammatory reactions and harbor bacteria.
