Fat Transfer? Worth it? Had Double mastectomy with reconstruction. Interested in adding fat to the top
fat grafting
Jun 17, 2019
Fat grafting is a great tool to help camouflage the edges of the implant, correct a contour abnormality, or improve volume/shape in reconstruction. If you are happy with your current reconstruction, then it’s not needed. From the photos you are a thin person already, and so the trade offs from fat grafting are 1. its another surgery- anesthesia, recovery time, etc. 2. not all of the fat survives, on average 60% depending on technique, 3. there is potential…
5 weeks post mastectomy with implant breast reconstruction. Can this be an implant ripple?
Rippling
May 21, 2019
Good morning. You are still very early post surgery, so please be patient. I’d recommend going back to your surgeon with concerns.. from the photo this does not look like rippling. rippling would also be unusual with siltex implants. More likely it’s an irregularity from the mastectomy itself, a drain, or edema. These often soften over time, so your surgeon will likely suggest you wait for 4-6 months. If it persists a revision with fat grafting would be …
Is this an ok implant job? And what can be done to fix it?
Post mastectomy asymmetry
May 18, 2019
Good morning,

Breast reconstruction is a complex process and although we strive to be done after one surgery that’s not always possible. I think you have a great outcome despite the asymmetry. Nipple reconstruction can be done, and is a simple procedure that can even be done under local, however the new nipple will not be arousable, and may lose volume over time. This is a normal result from nipple sparing mastectomy. Regarding the asymmetry, implants are hardly e…

Why are my reconstructed breasts different?
Tram vs diep
May 8, 2019
Thanks for your question. I would recommend following up with your surgeon. There are a number of reasons this might be the case, but would need more information. Did you have unilateral radiation ? This can make the skin tight and difficult to match softness side to side. Sometimes there’s also firmness from fat necrosis (dead fat) attached to the flap… and this can improve over time. Also, if your surgery were more recent and you are still r…
Breast expander ports moving. Should I be less active?
Mobile tissue expanders
May 7, 2019
Good morning, thanks for your question. It is not uncommon for Tissue expanders/shaped implants to rotate if the pocket created is larger than the implant or it isn’t sutured into place. With a tissue expanders this is typically not an issue as you are already scheduled for a second surgery. Any asymmetry from a moved or rotated expander can be corrected at that time. As always, this should be discussed with your surgeon.
What surgical plan would you recommend for unilateral LCIS diagnosis?
LCIS reconstruction
Apr 30, 2019
Good morning. You present with a somewhat complex case given that the mass is large- >5cm on MRI, but your biopsy thus far is LCIS. I’m also confused that caption says suspicious for DCIS. I would recommend a coordinated approach with your breast surgeon and reconstructive surgeon. They can give you more details with a full review of your history and physical exam. My general preference if you are already leaning towards mastectomy is for nipple…
What can be done to remedy implants flipping after bilateral mastectomy for breast cancer?
flipping implant
Apr 29, 2019
Thanks for your question. I would recommend that you return to see your plastic surgeon. As there are no photos it is difficult to tell what you mean by flipping. Implants can be round or shaped. If a round implant flips it’s difficult to detect unless you feel the seal on the back of the implant, but this would be unlikely to create a visual abnormality. Shaped implants, on the other hand flip or rotate at least 20% of the time and if this happens it’s an…
