This is a common issue and can be explained a few different ways. The firmness and discomfort you are explaining seem to be very consistent with capsular contracture. This, I believe, is secondary to an initial problem of incomplete release and subsequent lack of lower pole development. The nipple is not center in the middle of the implant. I commonly preform the lift at the same time if there is adequate tissue to support the viability of the nipple. Typically if the implant is below the muscle then there should be enough blood flow to give a good breast shape and nipple reposition.

Alloderm will become part of your tissues with time and in the short term this will fix the thin area as this is likely an excellent repair...because thin tissues where the main problem. If the tissue thins out again perhaps implant downsizing or removal should be considered. You may never have to consider the later.

Definitely too early to tell, especially if sub muscular. It will take at least 8-12 weeks to realize the position once the muscles relax and allow the implants to "drop" into their pocket.

This commonly occurs when an implant is heavier than the inferior pole support of the soft tissues. These tissue expand...and the distance from the nipple to edge of inferior implant increases. To treat this the soft tissue needs to be excised and the implant supported or downsized.

Two weeks should be fine, as you will be on prophylactic antibiotics and can delay your augmentation if a rare infection or issue develops (rare). Uncomplicated wisdom teeth extraction only causes a transient bacteremia.If any of my patients undergo dental work soon after an augmentation then i ask they use some peri-procedure antibiotic prophylaxis.

It is definitely too early to worry about implant positioning...especially in a sub muscular position. When the surgery is done...the muscle tone is high and it takes 8-12 weeks for the muscle to relax and for the implants to descend into the pocket which was dissected. Glad you are otherwise well...Its important to moisturize and care for your incisions. The itching is part of the healing process. If it gets too bad then benadryl can sometimes help...also cessation of narcotics use can help.
