You do have some degree of ptosis (or droop) to your breasts at this point. It is likely adding the implant will exaggerate that degree of droop. I would recommend the lift to increase the longevity of your result.

This can happen over time. It sounds like your surgeon did the right thing checking you out to make sure there was nothing serious going on with your implants. Sometimes they move a bit but are not a problem.

Based on the photographs you have provided, I would not recommend a lift. The pictures are a little limited so we would need to do a more complete exam to determine for sure.

This is fairly normal. It’s possible that the one side moves a little more to the lateral aspect of the pocket when you are lying down.

This is possible and happens somewhat frequently. It is fine to use a modest size implant to get the upper fullness without making a huge breast. Often in this procedure, it is necessary to redo the incision under the breast to get the best shape. This allows access to place the implant.

I’m not sure you really have options other than an implant if you want larger breasts. It does not sound like there is fat to transfer. You can work with your surgeon to find an implant size/shape that will be help with your goals.

No need to worry. This appears to be a normal vein that does not require any treatment. Just keep an eye on the area, it will likely stay the same.

It sound like you may have capsular contracture on your left side. The best advice is to have an exam by a plastic surgeon to determine what is going on and what can be done to help.

If this were earlier, I would suggest the bump is likely a suture trying to poke through the skin. That is unlikely this far out. I recommend placing warm compresses on the area to help it come more to the surface. I imagine it will resolve on its own.

What you are describing does not sound like capsular contracture. Different positions can make the implant more noticeable which sounds like what you are feeling.

With the amount of droop your breast currently has and your project of losing more volume, your best option will be an implant with a more substantial lift.

There can be a range of sizes that would work for you. I like to base the implant off your base width measurment (the width of the breast). We can then work with different profiles if you want more or fewer cc’s.

To me, you have asked about 2 different procedures. A FDL tuck involves a vertical incision in your midline but toward the breast while an extended involves a longer low incision that curves around to your back. I think you are a good candidate for the extended. I would not offer the FLD.

These A1c levels are generally below what is considered diabetic so that part may be ok. My more experienced patients require a bit more diligent clearance process to ensure good candidacy for surgery. So long as those come back clear, you can be a candidate for surgery.
