You have a constricted lower pole of the breast rather than a tuberous breast. This is very common for breast augmentation patients. There are many different ways to approach this issue surgically. No specific way is better than the others which is why you are getting multiple opinions. Breast surgeons generally will remove areas diagnosed as PASH. Your plastic surgeon will want you to have that area removed and totally healed for a few months prior to proceeding with augmentation. In your case, a lift would be needed to get your nipples/areola back in a more ideal position. Augmentation alone would place the implant higher on your chest with your breast tissue still sitting a low. Your plastic surgeon will use techniques specifically for your shape of breast to help it round out over the implant. Puffy nipples over implants frequently are not the most ideal look so most patients elect to have it corrected as much as possible. There is no great way to make outward pointing nipples point toward the middle. A breast augmentation can decrease the gap between the breasts but will amplify the outward pointing look. Many plastic surgeons have imaging programs (crisalix, vectra) that can help you visualize your post operative result that may help guide your decision. The severity is that your breasts are absolutely not tuberous at all. There is no such thing as normal when it comes to breast other than all natural breasts are normal. Some people have sagging earlier than others. Based on your comments, I would recommend a breast lift. You could also discuss what adding an implant would add – if you wanted more volume. You should be assured that you have completely normal breasts that are not shaped weirdly. You might have a tiny bit of tuberousness. If you wanted increased size, a breast augmentation with manipulation of the breast tissue you have could give a result more in line with the breasts seen online and TV. I would have you wait a minimum of 6 months after surgery to do the implant exchange. You will likely get answers from 3-12 months for this question. There is no absolute here. It is extremely difficult to get insurance to cover any breast augmentation procedure. During puberty many breasts appear tuberous as they are coming in as a normal part of the breast growth process. This is a well described but less common incision for most surgeons these days. The most common incision is the one under the breast. Transaxillary can definitely be done upon patient request. The most common reason it is less frequent is because it is harder to see inside when doing the dissection. Many surgeons overcome this by using a special camera to help with the procedure.
