Your initial results look pretty good.  In some cases, additional spontaneous skin tightening can take up to a year.  It occurs very slowly, so be patient.  You age is also a significant plus.And finally, try losing some weight (at least 10-15 lbs or more) and pumping iron to build up the pecs.  Larger, bulkier pecs may aid in taking up any slack of the skin.Elliot W. Thanks for your photos.  You appear to be middle aged and your skin probably has some innate laxity, all of which makes any revision surgery more challenging.  You do appear to have a crater deformity involving tissues below the nipples, rather than involving the nipples themselves.  The “good news” is that you appear to have residual fat above the nipples as well as lower chest and upper abdomen.  There is also excess fat on the lateral chest.  These adjacent areas of residual fat can be…

Yes, a revision is certainly possible.  It appears that you have residual fat and excess skin.  In order to achieve the results you desire, more than likely additional liposuction will be required both from the front and the back.  And then skin excision would be needed to tighten the skin and that, too, would have to be performed with extension around to the back.  Best to discuss these ideas with your surgeon.Good luck!Elliot W. You would be an excellent candidate for liposuction of the neck, which will contour the area under the chin and then extend under the jawbone.  This is a fairly quick and very safe operation and the results are apparent fairly quickly.  You absolutely do not need any skin tightening of any kind — you skin will conform to the new neck contours after surgery and it should tighten as well.Good luck!Elliot W. You did not provide a photo so I can only generalize.  Upper eyelid surgery on a male should always be done conservatively.  Men usually have lower eyebrows which contribute to apparent upper eyelid excess skin.  The incision is hidden in the natural fold crease of the upper eyelids.  It heals quickly.  You should be able to return to sedentary work within one week — perhaps less.  You should not need any camouflage makeup at any time.  If done properly, the shape of your eyes should not ch…

Most methods utilize permanent sutures to hold the ear cartilage in place.  If these sutures have broken through the skin, they may provide an entry point for infection.  In essence, one should consider removing them.  It would be best, if possible, to return to your original surgeon.  Finding another surgeon in the UK may be difficult.Good luck!Elliot W. It would be advisable to remove the nipple rings at least a few days prior to surgery so that the holes of the piercings have closed.  You can re-pierce the nipples some weeks in the future if you desire.Elliot W. Jacobs, MD, FACSNew York City
