I am planning on having a full facelift in the near future. Are such procedures typically done in office or hospital/clinic?
Nov 10, 2015
A full facelift, including eyelids and neck repair can be
safely performed on healthy individuals in an office-based surgery or
ambulatory surgery center. The location for surgery depends on several factors,
first, is your health and what type of procedure is planned, including type of
anesthesia. Second, make certain that the office facility is accredited by an
inspecting agency such as Quad A-SF. Finally, inquire about your prospective
surgeon’s experience in performing rhytidectomy. The...
Why are patients bent over after Tummy Tuck? Is it the muscle repair or incision?
Why are patients bent over after Tummy Tuck? Is it the muscle repair or incision?
Nov 10, 2015
Patients often are bent over for a few days following
abdominoplasty surgery because of the tightening of the deep layer muscle
fascia and the removal of lower abdominal skin. Some patients find that it is
more comfortable to take any stretch off their lower abdomen after surgery for
a few days.

This is a short-term issue and resolves nicely.

With Sientra implants now unavailable, are there increased risks using a gel filled implant vs. a highly cohesive gel implant?
Nov 10, 2015
All FDA-approved implants have a type of cohesive gel
versus older-generation implants from the 1980’s that are no longer sold. The
anatomically-shaped implants have a firmer gel that retains its shape in the
body to act as an internal breast form (Allergan 410 and Mentor CPG). Round
implants also shape the breast, but also fill out the breast to restore volume
lost after pregnancy.

The safety profile of round and anatomically-shaped
implants is well-characterized. Feel free to discuss...

Do these sound like symptoms of capsular contracture?
Do I have capsular contracture after breast augmentation?
Jan 2, 2015
It has been my
experience that patients like yourself have both tight chest wall muscles and
irritated sensory nerves that come out of the chest wall in the breast region
after a bi-planar breast augmentation. Musculoskeletal
discomfort of this type is improved with stretching. Nerve discomfort typically
goes away over time without treatment. Capsular
contracture involves hardness in the breast after implant placement. If your
breasts are soft, you may be having musculoskeletal or irritated...

Seroma after abdominoplasty
Jan 2, 2015
I am sorry to
hear that you are having difficulties. Fluid accumulation after an
abdominoplasty that was designed to eliminate drains by using quilting stitches
can occur. This becomes annoying, when you stand up and fluid drains through
the skin.You may want to
discuss with your surgeon if you need a drain placed and wound culture of the
fluid. In my experience, this may resolve the problem. Good luck!

Dealyed healing and lumpiness after breast reduction
Jan 2, 2015
You sound like a
healthy individual who has had some problems of tissue blood supply after a reduction
mammoplasty. Delayed healing and lumpiness in the deeper tissues are rare, yet
manageable.Typically skin healing in
the nipple and areolar region occurs soon after surgery. The lumpiness may be
fat necrosis, where the fatty tissue of the breast loses blood supply after
surgery and hardens. These lumps can be evaluated with a diagnostic ultrasound
exam and removed by an open biopsy. Good luck!
Do I need a breast lift or just implants?
Revision options and strategies for a patient with size asymmetry and short nipple-to-fold distance
Jan 2, 2015
Your history
indicates a former trans-axillary breast augmentation, with implant removal
(location of incision not specified). The photos show moderately severe shape
and size differences between your breasts and a short nipple-to-fold distance.
You indicate that after implant removal that the lower breast region became
more constricted and is not visible now. This is a challenging situation of
re-operative breast surgery. It has been my
experience in patients like you that the lower breast...
Juvederm - what are these white bumps around my lips?
Bumps after Juvederm
Apr 8, 2008
Juvederm is a clear gel, and is commonly injected into the lips. This is likely the natural appearance of the skin of your lips, it is simply now more visible since the lips have been augmented. Follow-up with the doctor who performed your injections to be certain the condition is benign, and not an unrelated viral or other skin condition.
Is mesotherapy FDA approved?
Mesotherapy is not FDA approved
Mar 28, 2008
There is no conclusive, authoritative information that exists (such as unbiased, peer-reviewed clinical studies that meets the standards of an accepted medical journal) that measures both the efficacy and safety of mesotherapy.
Some doctors will tell you mesotherapy is FDA approved because the individual ingredients that are compounded (mixed together) to make mesotherapy may be FDA approved for unrelated purposes, but the FDA hasn't approved mixing and then injecting these ingredients...
Botox side effects - why did Botox give me a lazy eye?
Ptosis is the most common Botox side effect
Mar 28, 2008
What you describe as a “lazy eye” is technically called ptosis. Although complications from injections of Botox Cosmetic® are rare, ptosis is the most common complication reported. It results from a combination of the injected dosage and your own anatomy’s response to treatment.
Ptosis is benign, meaning it does not affect your health or present any danger to you. Your appearance will improve over time as the effects of the injection begin to diminish, within 90...
Does Botox treat migraines and wrinkles at the same time?
Botox has more than one approved indication
Mar 27, 2008
The use of botulinum toxin (Botox®) for the treatment of migraine headache disorders has been publicized in both the lay press and the medical literature. This represents a novel and sometimes effective approach to treating this disorder, as opposed to other types of drugs. Botox® used in such treatments would constitute an off-label use of the product.
Allergan, the maker of Botox® has several approved indications for the product, both for functional disorders (Botox®) and...

Hyaluronidase for dissolving excess hyaluronic lip fillers
Mar 13, 2008
The use of hyaluronidase for dissolving excess hyaluronic fillers (such as Restylane or Juvederm) is a not uncommon. Other approaches to improve excess filler or lumpiness includes insertion of a hypodermic needle to extract (aspriate) the product.

Before a physician uses either of these approaches it is essential to evaluate the lump to make sure that it is specifically excess product and not a granuloma or other type of mass that may potentiall occur after filler injections.

Nov 5, 2007
Cosmetic injectable therapies are a rapidly growing segment of treatment that many people are quick to embrace but don’t yet fully understand. While Botox and dermal filler injections are not surgical, they are still wholly medical procedures that require the experience and oversight of a physician specifically trained in cosmetic procedures of the face. A qualified physician is key, not only to ensure safety, but also to ensure an aesthetically pleasing outcome.
