I can see an apparent bump on your photos, but I cannot diagnose the cause without an in-person examination. It looks like you may have damaged and/or displaced the upper lateral cartilages, which is waht shapes the flexible part of your nose above the tip. Probably fixable, but with a higher risk of an unsatisfactory result than a simple rhinoplasty. Perhaps that is why your surgeon was not keen to fix it. Narrowing the nose, without lowering the profile on the side view can be done by cutting and repositioning the bones. It also looks like your tip would need to be slimmed to match, by adjusting the tip cartilages. I normally do the nostril reduction as a no-cost touch up under local anesthetic after everything else has settled at about 6 months post surgery. So long as everything is done in moderation, should be a very natural looking improvement!

Looks like you are developing PIH from sun exposure to the area that still has some healing going on (and therefore inflammation) from your recent rhinoplasty. This is an issue with sun exposure after any surgery, particularly with skin that tans easily. Often seen in Asians too. Based on the 3 photos, it seems likely that significant slimming of the tip would make the width of the nose above the tip (the dorsum) more noticeable. Photographs cannot show the thickness of the bone, or the position of the junction between the boney part of the dorsum and the cartilage. These are the two factors which determine whether your hump can be re-contoured with rasping only. It would also be helpful to see the side view of your nose from either side. If sthe latter, lengthening the nose would normalize the alar base position in relation to the rest of the tip. I have done abdominoplasties as an outpatient procedure, with a surgicenter 23 hour stay and as a hospital in-patient procedure with a 24 hour plus stay. I always place a pump to deliver local anesthetic into the muscle for the first 3 days or so.
