The typical options we offer patients are either breast reconstruction with your own tissue (e.g. DIEP flap from the abdomen) or similar procedures and implant-based reconstruction. The decision depends on patient preference, prior radiation, and body status, etc… I would not recommend reconstructing the breast with your contralateral breast.

In a patient with large breasts (a lot of skin), no matter what type of reconstruction they choose a majority of that skin…

In my practice, I prefer to use an acellular dermal matrix product (such as Alloderm or FlexHD) in conjunction with tissue expanders for breast reconstruction. This allows the expander to sit in a more anatomical location while partially releasing the pectoralis muscle. Expansion can be completed faster and the overall result I have been pleased with. I personally would not recommend fat injection to the lips as it can be unpredictable in the amount of fat that lives. The down side is that the blood supply to the lips is so great that it is easy to over correct the lips leaving an undesirable result. There are fillers that patients can try, they are not permanent, and if the patient likes the result there are other permanent options. Hope this helps!

We have learned that headaches can be generated by peripheral triggers (e.g. sensory nerves in the head and neck). When I treat patients for surgical treatment of migraine headaches, I first look to see if there was any trauma (or previous lacerations/incisions) in the area of where their pain starts. If so, then I treat the appropriate nerves causing the pain.

It all depends on where the incisions are located and the type of symptoms patients are experiencing…

Botox to treat the 11's needs to work on the muscle that runs from the nasal side (medial aspect) of the eyebrow to about the mid eyebrow where you may feel a notch. If you only inject between the eyebrows you probably will not temporarily paralyze the whole muscle unit and will not be full pleased with your results.

Whether Botox is diluted more is probably not as important as if the injection was too superficial. When the liquid is injected that volume will cause a "welt" and is more apparent if too superficial. Either way, it should go away quickly. If you had a small area that bled due to the botox you may have a small area that will bruise and take some time for the bump to go away as the blood dissolves. The good news is that it should not be permanent.

Botox for migraine can be variable in response not only where it is administered but also depending on what is causing the compression. For instance, when I decompress the frontal trigger site there is typically bone encircling the nerve which needs to be released and this will not be treated by botox. Therefore you may get partial relief but not full. This is among many factors of differing responses to botox therapeutic. I hope this helps.

When we perform fat transfer there are several factors that influence the overall survival of the fat transferred. One important factor is blood supply in the recipient area. The newly transferred fat must establish a new blood supply and if there is too much motion in the area this can disrupt the newly forming blood vessels and lead to less fat survival. This is just one of many factors involved.

We have learned through our work on "surgical treatment of migraine headaches" that some of the superficial nerves in the head and neck can lead to headaches. This is why our surgical and in some instances Botox works to relieve headache pain. Likewise, if you inject the area with Botox and caused some inflammation/irritation around the nerve this can also lead to headache pain. This should resolve shortly as the inflammation subsides.
