I think you have a very good results for the mount of filler used. I totally agree with you that the cost is high considering it only last 6-9 months. You may want to consider a permanent filler like Bellafil. the initial cost is slightly higher but considering the duration it lasts it become a lot cheaper. Neurotoxins like Dysport or Botox can spread and affect the muscles that moves your eye ball and as a result cause double vision or the vision to be off. It can take up to 4-6 weeks to recover but it usually disappears. you can try to cover one eye to see if it corrects or see an optometrist

On the Average the glabella, and forhead require 20 units each some patient will require more or less. The crows feet usually take 10-12.5 units on each side

You are very young and because of that you have to rule out the cause for the hair loss. I would recommend to see a hair surgeon and have them evaluate and perform an exam on your hair may have to order some labs to determine the cause first before exploring transplant issue.

Bellafill is a thick Volume filler that is used a little deeper than fine lines. For fine lines, I would try Restylane Silk, or Boletero.

It really depends on the dilution factor and how strong is your muscles but for the most part 3-5 shots.
