What would happen if I didn’t get the lift and only got implants for now?

I fully agree with the recommendation of your surgeon to get the best result. You have extreme ptosis and breast asymmetry and a staged procedure with bilateral lift with a small right breast reduction, followed by a second procedure for implant placement will give you the best shape and symmetry.  Implants alone would result in a poor result.
