Yes, it is safe to leave the deflated tissue expander until your next surgery. However, why would you not want it replaced? Keeping the skin envelope expanded is crucial to an aesthetically-pleasing reconstruction. Also, on the right (radiated) side, I would strongly recommend an autologous tissue reconstruction.

From the single photo provided, I would recommend bilateral mastopexies with fat grafting to the left (radiated) breast. I would try to stay away from breast implants, given your history of radiation therapy. The other option would be to reduce the right breast and lift the left breast (mastopexy) to obtain symmetry.I recommend a consultation with a board-certified plastic surgeon who does a lot of breast reconstruction in your hometown.Best of luck.

With unilateral capsular contracture, I would not recommend removing and replacing both implants with saline. Unilateral capsulectomy with implant exchange is a reasonable approach to this problem.If you decide to replace both implants, I would recommend switching to textured (as opposed to smooth) gel implants, for a lower capsular contracture risk.Wishing you all the best!

Thank you for the photos you submitted along with your question. Yes, this does look like axillary breast tissue, and yes, it can be excised. This might even be covered by your health insurance, but I would submit it to your insurance carrier for pre-authorization/pre-determination to be certain. If your insurance does not cover it, it might cost somewhere in the ball park of $3000, including anesthesia fees, facility fees, and surgeon’s fees.

I encourage breast augmentation patients to refrain from any strenuous exercise for at least 2 weeks. No bouncing, such as running, jumping rope, etc for 2 weeks. Then, once you resume those exercises, I stress the importance of a very supportive jog/sports bra.No pectoralis muscle work-out or upper body strengthening for 4-6 weeks, depending on the patient. Everyone heals differently.You could resume lower body workout at 2 weeks.Wishing you the best!

Wow. I am sorry to hear your daughter is going through this at such a young age. I definitely understand your concern. Most often, insurance will cover this type of surgery for these severe symptoms. You would have to see a plastic surgeon and obtain pre-authorization prior to surgery. As for her age, I believe she is a bit young to undergo breast reduction surgery. Yes, at age 39, I would encourage you to get a mammogram before any type of breast surgery. The ACS recommends annual mammography beginning at age 40. But, due to the fact you are scheduled for breast surgery, getting the mammogram at 39 is recommended. It is good to have a baseline image of your breast tissue, prior to any surgical rearrangement of tissue.Best of luck to you,

Not necessarily.  A drain may only be required for 24-48 hours after a breast reduction, so if it fell out on POD #3, it may be just fine.  Drainage patterns or amounts are different with each patient.  Some people drain more than others.  I usually go by volume, ie, a drain is ready to come out when it is draining <30 mL/24 hours.

I do a lot of breast surgery, including reductions, and my rule of thumb is as follows:  If I remove over 400 grams from a breast, I leave in a drain for 48-72 hours.  If I remove LESS than 400 grams, I do not leave a drain.  With a smaller breast reduction, a drain is not necessary.  With a larger one, the drain is necessary.

The post-op compression garment is very important after any liposuction surgery.  It must be worn 24 hrs/day for a minimum of one month to achieve the best results.  At 2 months post-op, you may be somewhat swollen, but generally all of the swelling should be gone by 3 months.  Some of my patients wear the compression garment for the whole 3 months, in order to achieve optimum results.

I feel certain that a fair amount of skin was taken at the time of your mastectomy (since you didn’t have immediate reconstruction), and therefore, you will require the tissue expansion process. You will end up with a more aesthetic outcome if you undergo the two-stage reconstruction [tissue expansion for 3 months followed by exchange to gel implants]…

It’s difficult to say whether your plastic surgeon could have done more without seeing your pre-operative photos. From this photo, it appears that your nipples are at a decent level. One thing you DON’T want is…nipples that are too high! That is a very difficult problem to fix. Your nipple-areolar complexes should be located at (approximately) the half-way point between your shoulder and elbow. Yours appear to be SLIGHTLY lower than that.
