It's difficult to tell from the one photo what would be best. I would want to hear your goals for surgery, but I think an implant would probably be sufficient to improve the fullness and appearance of your breasts. I would probably agree with the other surgeons with whom you have consulted.

I am assuming you would rather avoid the scars, but I think that a circumvertical mastopey would be necessary to lift you nipples.
I would need to see your preop photos to see what your breasts used to look like. It is possible that the additional weight of the implants has led to some of the sagging. If you want to reduce the fullness in the side of the breast, you would probably need a narrower / smaller implant

The left breast does look larger, and the nipple position is also slightly different. It is possible that this is due to your anatomy, but without seeing the before photos it is difficult to tell. I am assuming you have the same sized implants in both breasts. If your goals is to increase the volume on the right, fat grafting might be an option. I would recommend you discuss your concerns with your plastic surgeon.

With many operations, there may compromises based on what priorities. If you want to reduce the areolar diameter, you would need an incision around the areolae. This may limit the implant volume that could be placed. You may or may not benefit from the vertical incision, but if the benefit is small, you may prefer to avoid the scar and accept the results without the additional scarring.

Based on your photos, it looks like you could improve the appearance of your breasts with different procedures, depending on your goals. If your goal is to look like the women in the photos, you would need more volume. It looks like all of those women in your wish photos have implants. I'm sorry you're not happy with the results.  It is still a little early post-op and your implants will likely settle over the next few months.  If you are still not happy after that time, you should definitely discuss your concerns with your plastic surgeon.

It is difficult if not impossible to predict a specific bra cup size when performing a breast reduction.  Bra cups are not standardized, so one manufacturers C may be another's D or DD.  In general, I tell patients that they will be smaller, and more proportionate to the rest of their body.  You should try not to focus on a specific bra size.  If you are happy with the size of your breasts and your bra says D, does it matter?  
How can I achieve a look where contours of the implants are quite visible?

The look you are trying to achieve would require relatively large, high or extra high profile implants, placed over the muscle.  The asymmetry in nipple position may be more prominent with implants.  Placing the implants you want to achieve your look, however, will preclude the ability to elevate the nipple and areola.

Rippling can be visible when there is insufficient soft tissue covering the implants, and/or there is a discrepancy between the pocket size and the implant size.  In your case, it looks like you probably have insufficient soft tissue covering the implant.  Fat grafting would be an option.  Off the shelf fillers can also help, but may be considerably more expensive if a large volume is needed.
Will my breast sag after explanation and can stretch marks appear?

It does not look like you would have much sagging of your breasts after removal of your implants.  If you don't have stretch marks now, they will not appear after the implants are removed.  As for downsizing, the there are smaller implants, but not much.  I would probably ask you why you want your implants removed and what are you trying to achieve?
Can I just start with implants then get a lift later?

You could certainly start out with implants only.  This would definitely give you more upper pole fullness.  The results won't be optimal, however, but it might convince you that the scars would be worth the improved aesthetics.

Swelling after a tummy tuck and liposuction can take months to resolve.  I would give it more time.  The belly button shape may also change over time.  I recommend you discuss your concerns with your plastic surgeon.

Pain after breast reduction usually lasts between 4-6 weeks.  Pain is very subjective and can vary from patient to patient.  I would check in with your plastic surgeon to make sure that you are healing normally.
