200 CC will put you into the D range for sure, have you considered staggering the size of the implants to achieve better symmetry? Just something to think about, otherwise go same size implants and be happy with the added volume

Are not in a terrible position, look at your preop pictures, the way your body developed your nipples are far lateral towards the outside of your chest, so this was going to happen to some extent regardless of implant chosen or surgeon performing procedure. Should have been a conversation before surgery, at this point, downsizing will help, for sure the LEFT is not going to be fixed by downsizing along, imagine where the implant would need to be in order your your nipple to be centered, it ...

Say that is not a dogear, that is flank fat that was not aggressively removed with liposuction or cut out during the TT portion of the procedure.

Question for your hematologist or internist whom is managing your meds. You will also need to find a plastic surgeon comfortable performed the surgery on lovenox or however it will be performed.

You will end up with a waterfall deformity, which I guess if you are ok with that vs the lift scars is ultimately up to you! But clearly not the "ideal" option for sure

Things being common, likely spitting suture(s), just needs to be removed and the area should heal without any problems, but you should see your surgeon or his nurse

Post op is a little early to be judging results, you still have some swelling in your breasts, which will take some time, the implants are likely going to have some settling to do, having a 25cc asymmetry is common for most woman, realize it may be less or more than 25cc difference and silicone implants only stagger 25-50cc in size, so you will have some asymmetry, the goal is to make the asymmetry less, but they will likely never be "twins".

Dependent on the area you live in. Going rate prob on the low side in the US would be 10k ish, high side would be 20k, 17k your prob right in the middle depending on exactly what procedures it is you need.

Surgeon is telling you in advance he will likely not be able to remove all of the skin from the low transverse(TT) incision to above your current BB-meaning you dont have enough skin/laxity, so when he pulls the skin down you will have a small vertical scar from the area where your BB used to live, this is a better alternative than bringing the transverse scar higher towards your BB, which some surgeons may suggest.
Severe dermatitis (rash) after tummy tuck. Has anyone seen this before?

Definitely seen it in a few patients, as stated below, could be the prep solution, could be the steri strips or adhesive used to keep them in place, need to discuss with your surgeon, likely need topical steroid cream +/- oral steroids.

You have a nice result, doesnt appear to look to "fake" or unnatural, but ultimately that decision is up to you, realize downsizing would require new implants likely atleast 75-100cc smaller than you currently have. 
Is this already grade 2 ptosis? Which surgery would be best for my breasts?

Will likely give you the result you are looking for. Some will be happy with mastopexy alone, but most woman desire the upper pole fullness that can only be achieved with an implant
