Thanks for the question. There are many downsides to a free nipple graft, loss of sensation would be one of them. I try to make patients proportional to their body with a breast reduction which tends to look nice on most patients. I would recommend a consultation with a board certified plastic surgeon to discuss this further.

I tend to use sizers with a bra to help determine what size implant to choose. I also use your body as a guide and take measurements. These measurements then allow us to choose from various different size implants with differing profiles as well. I think working with your surgeon is also critical to your success in achieving your goals.
Implant height is very rarely discussed here. Would anatomically shaped "tall" implants be appropriate

Thanks for your question. You have done some great research as to the different types of implants as well as the various techniques we do. Shaped implants are a great way to get a natural look but you can also get a very natural look with a round implant. I would encourage you to find a Board Certified Plastic Surgeon whom has experience in both shaped and round devices. This way he or she can help you make the best decision to achieve your goals.

Thank you for your question. Typically having bottoming as shown in your photos is going to require a revision procedure. If you were my patient, I would place an ADM or Strattice along the lower pole to hold the implant in place. I also typically in my patients try to put in a slightly smaller implant placed in order to have less stress or weight on the lower pole of your breast. This is a large implant. I would recommend talking again with your PS about this size. Perhaps even ask to see one and try that on with a bra so you have an idea of what that would feel like to have on your chest. Everyone size, shape, expectations are different of course. I like to use measurements of the patient to help choose a size that will work for their body and that works for them/their lifestyle and what they are looking for. Hope this helps,

Congrats on your new implants. I too like the TX Inspira implants. You are def still early in the post op phase and your implants will drop a bit more and more swelling will go down. I don't usually think about revision surgery for at least 6-9 months for size change as I believe changes are still taking place. Good luck to you,

Thanks for the question. Sorry to hear about your capsular contracture (CC). In my patients, I like to use textured implants as I believe there is less CC with this. I also use other techniques like capsulectomy/otomy and possibly even an ADM to decrease the risk of CC. Certainly finding a Board Certified Plastic Surgeon and working with one will be helpful to you. Submuscular position also lowers the rate of CC. Good luck to you,

Tissue based planning is the way to achieve a nice result. This refers to specific measurements your surgeon takes during your consultation and then chooses an implant based on those numbers as well as your anatomy, breast tissue, shape and size. Make sure to contact a board certified plastic surgeon to help you achieve your goals.

It is normal to have slightly more swelling on one side versus the other. Healing takes time and it may not be symmetrical. Check in with your surgeon as well and keep them posted at to your recovery.

Thanks for the question-there are some implants that dont need massage, so i would recommend checking with your surgeon to determine if you should be massaging them. with that said, 2 days post op, implants and breasts are typically firm and high.

Good question-we all have ways to determine implant size and shape. most of us use the patient breast width and how much skin stretch one has. these are 2 important variables that can help determine the right size implant. with a narrow breast width like you have, the size your are choosing may be reasonable. it is often helpful to the surgeon to have patients bring in photos of patients with similiar breast implant size and dimension. hope this helps,

Reviewing your preop and post op photos, you appear to have likely needed a breast lift. i dont think that 20 cc would account for the difference between the 2 breasts soley. it would be reasonable to keep the lines of communication open with your surgeon

Usually you want to wait about 3 months after you have finished nursing. You also want to return to your pre pregnancy weight and breast size as much as possible. At that point, you could be evaulated by a plastic surgeon.

Reviewing your photos it looks like you will have some skin necrosis along the central part of your incision. This is something that should be followed by your plastic surgeon.
