It would depend on what you mean by "tummy tuck". A small amount of elliptical skin and subcutaneous tissue can be excised under local. However, a traditional tummy tuck could not be performed under local for many reasons including, the in ability to obtain adequate anesthesia without reaching toxic levels.

From the picture submitted, it appears that the left breast might be tuberous in shape. It's hard to tell if the right breast is tuberous. The procedure I would recommend would depend on whether you were trying to correct the asymmetry, the shape of the breasts, the size of the breasts, or all three. If all three are of concern, then I would do a differential augmentation mammoplasty, and some sort of mastopexy on the left breast, depending on the physical exam.

From the pictures submitted, it appears that you are not a good candidate for fat transfer as a means to augment the breasts. Not only would there be a problem of obtaining an adequate amount of fat, but also there could be irregularities created at the donor site. I believe breast implants would be your best option.

Judging from the pictures submitted, I do not feel that any procedure other than an abdominoplasty (tummy tuck) would address your concerns and give you a good result.

We follow the guidelines of the FDA requirements, and recommend that patients under the age of 22 are not candidates for silicone gel implants.

Two weeks off of work is typically what we tell patients with administrative jobs. However, with the job description, we would recommend a month off of work. You may be able go back before then, however, plan on a month off.

Your best result would be with a standard tummy tuck. It appears from the pictures, that liposuction alone, of any type, would accentuate your skin laxity. It would be best to seek a Board Certified Plastic Surgeon.

If it has been a least a year since your weight loss and you still have little skin retraction, then I believe you will need a bilateral mastopexy with your augmentation. If these pictures were taken at the conclusion of your weight loss, you could consider waiting to see if you had any further retraction, but I would not be optimistic in that regard. In my opinion, a breast augmentation alone would result in a surgical outcome in which you would not be happy.

Yes, however your subsequent breast size may be smaller than your pre-operative size. How much smaller... depends on how much skin is excised and how much breast tissue you have pre-operatively.

More than likely, this represents capsular contracture. The contracture of the scar tissue around the implant is what causes the firmness and displacement of the implant.

At two weeks post-operatively, it is not uncommon to have the sensory nerves be affected by swelling and or surgical trauma. This usually resolves as the swelling subsides, which may take several more weeks.
