An IPL, which can target vascular lesions, is the preferred treatment. Since this hasn’t helped you, you might want to consider changing course to something that stimulates collagen instead. The devise that we have here is an erbium fractional laser (ER:YAG). It would take 2-3 treatments but would definitely yield results.

The most important factor is going to be avoiding any additional trauma to this lesion, including not only your hands but also sunlight. To specifically treat that scar you have a couple options, including some lasers and simpler treatment with a device called a hyfrecator, which creates electrical energy and is applied to the skin surface. All scars are treatable, it’s just a matter of time and cost. Regarding ethnic skin specifically, you’ll just need to make sure whoever is treating you isn’t too aggressive with whatever therapy they choose. A handful of laser treatments should do the trick, but a few decent chemical peels would also help. In our office we treat scars with the sharplight 2940 laser, along with the cynosure focus laser. The peels we prefer are made by skinmedica.

The problem you are experiencing is unfortunately extremely common in patients with ethnic skin. As long as you keep the area clean and avoid direct sunlight exposure, however, it should heal fine. In addition, a topical scar product such as BioCorneum will help minimize any lasting pigment changes.

Fillers can commonly migrate beyond the injection site. The area of your concern is close to a specific group of facial muscles that are close to the eye (the levator labii muscles). When fillers are injected near this area, these muscles can be pushed closer to the surface and become more pronounced. This should improve with time. Regarding the Vitrase, I agree with Dr. Kaplan. You do not have to wait, although I personally would wait till he is done growing so that the scar does not enlarge any further. There is always a risk that the hypertrophic scar returns following an excision, but typically there are measures taken (intralesional kenalong injections) to prevent this. Topical scar creams with silicone and sunscreen are another option which can help his scar in the meantime.
