Although it’s difficult to say without an in-person exam, it looks like your nipple position is good, your skin looks healthy, and ID expect you don’t need a lift at all. I also think a smaller implant will fit better in the pocket, as it seems much of you implant volume is in the upper pole.

Worst case scenario? nipples are a bit low after the procedure and you can do a lift later, possibly under local anesthetic even.

You have something we call an “animation deformity”, which is when you can move your implant and breast quite significantly with your pectoralis muscle, wrinkling the skin too.

You’d need a surgical revision to tighten the pocket and release the muscle more. Possibly fat grafting too.

I’d discuss this issue with your plastic surgeon.

It’s difficult to say with just the one photo, but it appears that just implants would be ok. A lot will depend on the look you’re going for (more natural vs. more rounded or “artificial”)

If you’re looking for a DD, you likely don’t need a lift.

It appears you’d best benefit from a lift. If you don’t, you’ll risk getting a waterfall deformity (breast tissue sits below the level of the implant).

One “workaround” in your circumstance would be to use oversized implants, possibly a classic dual plane. But understand that this will leave you with large, low positioned breasts. Also they’d likely be much larger than you like.

Plastic surgery is often a trade-off between scars and shape/form.

From the pictures and your description it’d appear you need a revision. The implant size seems to be quite asymmetric. You also have a waterfall deformity, which is when the breast tissue sits below the level of the implant. This may be due to the tuberous breast, or that the implants have not descended to the correct position, or both.

I’d agreed that smaller implants would help. You also would need a release of the lower portion of your pocket, or even potentially a breast lift.

It appears your implants sit slightly low on the chest. I’d recommend a repair of the inframammary fold and lateral pocket, and additional support with GalaFlex mesh scaffold.

As far as implants go, you may not need to go much larger, but I agree that a higher profile implant will achieve what you’re looking for.

Yes, this can be revised.

Your areolas were likely quite wide prior to the surgery, making it difficult/dangerous to remove all of that areolar skin initially. Now that the skin has stretched, a revision should be pretty straightforward. I’d discuss with your surgeon, he’ll be able to give you options depending on how extensive of a procedure you’d like. A simple scar revision can be done in the office under local anesthetic. Otherwise a repeat lift could be done as well.

Given your measurements, I’d generally recommend a lift. However if you at getting larger implants, it may not be necessary if you are willing to accept a lower nipple position.

Yes, it is possible. The surgery required for this is a bit more complex than a simple breast augmentation.

In your circumstance I would perform a hybrid breast augmentation (internal bra, modified dual plane, fat grafting) along with some additional internal work to ensure appropriate expansion of the lower pole of the breast. I’d probably also recommend a lift, particularly on the left side (possibly both), to improve upon symmetry.

To start off you should know that your breasts look absolutely normal. Nipple position can vary amongst people, some closer in, some further out. Usually the nipple lands about midway on the breast, or slightly further out.

From your picture it’s very difficult to assess, but yes, you may have a mild form of tuberous breast. This is nothing to be ashamed of, and in fact, quite common.

In your case I’d suggest a hybrid breast augmentation. It’s a procedure specially designed to attain “bra-less” cleavage, providing strong support for the implant and designing close cleavage lines that look good in and out of clothing.

Although difficult to assess from the pictures, it appears you mat benefit from a breast lift and pocket revision. Possibly the addition of an internal scaffold like GalaFlex to properly support the pocket.

I know you’ve discussed it with your surgeon, but I’d attempt at least once more to describe your concerns and ask him/her to fix it for you. If not, get a consultation with another surgeon and express your concerns.
