How long after vertical breast reduction can you have a mammogram done? When can you sleep on your stom
mammogram after reduction
Oct 24, 2018
Hello,
Thanks for reaching out with your question. If you are due for a mammogram, it should be done before breast reduction surgery as it is important to know if there are any areas of concern within the breast before embarking on the breast tissue removal/rearrangement that takes place with reduction surgery. If you’ve already had surgery, I would say wait at least 2-3 months for the swelling to resolve and to avoid significant discomfort during the mammogram procedure itself. I…
Follow-up question: Bulge around belly button, advice?
Abdominal bulge
May 12, 2018
Thank you for reaching out with your question. It appears as though the bulge you describe is due to the contour/shape of the muscles and fascia that make up the abdominal wall, not a problem with the fat cells and skin superficial to those muscles that are re-draped over the abdominal wall during an abdominoplasty. This is either due to diastasis, where the rectus muscles separate at the midline and splay to either side, or extra fat tissue deep to the abdominal wall around the…
4 months post op tummy tuck. Will lipo be enough to take care of u-shaped budge or do you think Ill nee
Lipo vs. more skin excision
Apr 10, 2018
Hello, thank you for reaching out with your question. This far out from surgery, it’s not likely that the area you’re concerned about will change much with time. I’m not sure simply excising extra skin will fix the problem, as it looks like there is a pocket of fat cells beneath the skin that is contributing to the bulge. I think a sound plan would be to try liposuction first, as the skin over the area of lipo does tend to tighten somewhat. Then, once you’ve recovered from the lipo, it would…
Can someone tell me if I’m a great candidate for liposuction or tummy tuck?
Liposuction vs. TT
Apr 10, 2018
Thank you for reaching out with your question. Liposuction is a great technique for removing fat cells from problem areas, and after liposuction the skin does tighten somewhat in response to the removal of the fat cells beneath it. In the abdomen, only the fat cells on the outside of the rectus muscles can be removed with liposuction. The fat cells underneath the muscles are off limits for plastic surgery of any kind. A good way to tell how much is above the muscles is to squeeze the…
I still have breast tissue after gynecomastia surgery.
Gynecomastia surgery post-op
Mar 20, 2018
Thank you for reaching out with your question. I would expect you to still be quite swollen at just 2 weeks out from surgery, especially if liposuction was performed in addition to the direct glandular excision. It can take 2-3 months for swelling to completely resolve, and it may take even longer for the skin envelope of the chest to retract after removal of the tissue beneath it. Scars take up to a year to fully mature. You’ll see changes over weeks, not days. If you still have…
Do I need a FDL tummy tuck or is circumferential enough?
Tummy tuck after weight loss
Mar 20, 2018
Thank you for reaching out with your question and congratulations on your weight loss! It’s tough to say by just looking at pictures how much the FDL extension to a tummy tuck would help. Based on your photos it looks like you certainly could have it done in order to remove the maximum amount of extra skin, but I appreciate that you want to avoid that vertical scar. If you plan to gain a bit of weight, I would go ahead and do that first, then seek consultation with a board certified…
What are the best silicone implants for natural feel and movement?
Implant choice
Mar 20, 2018
Thanks for reaching out with your question. There are lots of choices these days for silicone implants. Lately in the plastic surgery literature there has been a lot of focus on a rare type of lymphoma that seems to result from implants with a textured shell, and while we are working to better understand this disease, some plastic surgeons have become more hesitant to use an implant with a textured shell. To this date, smooth implants have never been known to cause this and are a good choice…
Severe animation deformity, double bubble and/or bottoming out? Help! 3 years post op
Breast pain after implants
Mar 20, 2018
Thank you for reaching out with your question and I’m sorry to hear about the symptoms you’re having with your implants. An all-too-common side effect of placing implants under the muscle, especially in thin, active patients, is an animation deformity due to the pec major muscle pushing down on the implant when it is flexed. This typically isn’t a painful phenomenon, though, but the appearance of it can bother patients. I’m not sure what’s causing the pain and irritation you’re feeling, but…
I have tear drop silicone implants since 2000, so 18 years old. How can I tell if one ruptured?
Possible implant rupture
Mar 20, 2018
Thank you for reaching out with your question. It can be very hard to determine based on symptoms and a physical exam if a silicone implant is ruptured. A capsular contracture, which is tightening of the scar capsule around the implant, can cause the symptoms you’re experiencing, but this can happen regardless of whether or not the implant is ruptured. In these situations it is always best to meet with a board certified plastic surgeon to have him or her perform an exam and discuss your…
Is there a procedure to completely remove the areola, leaving only the nipple?
nipple reduction
Jun 21, 2017
Thank you for reaching out with your question. In theory it seems as though this SHOULD be possible, but I do not think it would work, nor would I recommend it. If this were to be attempted, the outer ring of breast skin would need to be bunched together significantly to match the inner ring around the nipple papilla, which would likely lead to a great deal of bunching and pleating of the skin. This may also affect the sensation of the nipple, and would surely affect your ability to breast…
What type of doctor will do a ventral hernia repair along with a panniculectomy?
hernia and panniculectomy
Jun 21, 2017
Congratulations on your weight loss! Your best result would be had by waiting until you’ve stabilized at what you think will be a healthy long-term weight before having surgery, but if you absolutely can’t live with the extra skin you could have it removed now, but do so with the knowledge that further weight loss will likely lead to a recurrence of the extra skin. I would recommend you consult with a board certified plastic surgeon about the extra skin of the lower abdomen, and…
What are the pros and cons of having a “drainless” mommy makeover?
Drains
Jun 21, 2017
Thank you for reaching out with your question. Drains and adhesives are designed to prevent fluid collections, or hematomas/seromas, after surgery. Drains can be an annoyance after surgery, but removing them is rarely painful if they are “hubless” (i.e. the diameter of the drain internally is equal to the diameter externally). Adhesives could obviate the need for drains if they could be shown to work just as well and if they are cost-effective. To my knowledge there has been no…
8 months post-op: Raised and hypertrophic breast reduction scars.
breast reduction scarring
Jun 19, 2017
Thank you for reaching out with your question. Scars tend to mature and fade over the first year after surgery, but occasionally they become hypertrophic, which we tend to see more commonly in young patients and those with darker skin complexion. Your scars do look somewhat hypertrophic, and it wouldn’t hurt to discuss with your surgeon ways to treat the scar. Taping, silicone treatment, and steroid injections are often used to help scars soften and fade. I don’t think it would be too…
Why is it that after 7 months of DIEP flap there is a bulge forming as if an muscles are still open?
bulge
Jun 19, 2017
Thank you for reaching out with your question. I’m assuming you’re talking about a bulge in the abdomen, which can be a long-term effect after abdominal tissue based breast reconstruction. It is more common in techniques that transfer muscle, like the TRAM flap, and less common in techniques that spare the muscle, like DIEP flaps. In the case of DIEP flaps, what is likely happening is weakening of the abdominal wall, possibly related to injury to the nerves that supply the rectus muscle,…
5’8″, 176lbs and my current bra size is 38D. What size implant should I get, to reach my goal? Also do
breast implants
Jun 19, 2017
Thank you for reaching out with your question. It’s tough to say what size implants would allow you to achieve your goal size by reviewing photos alone, but any consultation with a board certified plastic surgeon would involve a review of the size options and a fitting process where you would get a chance to “trial” different sizes to pick the one that suits you best. Based on your photos, it looks like you do have a degree of breast ptosis and may benefit from a lift. It’s important to…
