Time to massage! It pretty typical to the scar to tether in gynecomastia surgery. You need to massage, massage, massage. Grab the scar between your index and thumb and pull it away from your body. It will feel "stuck", which it is, stuck on the internal scar tissue. 3 minutes, twice a day. On the right, the implant is low, which (I agree with you!!!) is giving you a double bubble on your right. You can easily go down to a B, just have the breast reduced at the same time. Whether is cost more is up to the surgeon, as it does involve some extra work. Make sure the surgeon is confident about the nipple blood flow so you don't get nipple necrosis, I would NOT recommend a classic inferior pedicle reduction and a lift and an explant at the same time as that is high risk. In your case, the implants are 70cc different which creates symmetry. So you should add 70cc of fat to your smaller breasts on removal of the implants. Usually I go over by 20% to account for any fat loss. Remember ot have the PS FIX THE PEC MUSCLE on explant. Why PS have to be told to do this is beyond me, but you don't want a pec flex deformity. And consider maybe grafting both sides so you don't look too deflated.

A diastasis rectus can be repaired through an umbilical incision, during a tummy tuck, or through an old csection, for me anyway. I know docs have a hard time with the smaller incision but totally do-able. For you, you need some skin removed where it’s hanging over, save the umbilicus, and just see your muscle back together in their anatomically correct location. The answer is, absolutely. Tuberous is not a “contraindication” as we say in the business, in fact the aesthetic outcome is quite good as the skin will redrape and the areola gets smaller, and you can reset the inframammary crease. Use inframmary crease incisions only and as I always say, ONLY go to mastectomy surgeons with a less than 1% nipple necrosis rate!

I see this a lot with over the muscle... the implant bottoms out in to the breast, weighing down the breast and aging it. These implants need support. I would definitely put in an internal bra (subpectoral implant with ADM holding it up) and you would get everything to come up higher from getting rid of the drag on the skin alone. When the implant bottoms out this early, it is indeed hard to get it to stay up. I would do an underwire bra daily and see where you are after a month, but you certainly want to support it so it doesn't get any worse. In  your case, the bigger, looser, softer breast on the right got the smaller implant. The smaller, firmer, higher breast got the bigger implant. So the most likely scenario is that one breast (right) is just "more breast". The other breast (left) is just faker, firmer and "more implant". Leave it alone if it doesn't bother you too much, because you will always have an asymmetry after breast aug if you had an asymmetry before breast aug. OK, so you can take out implants under local, especially with an incision in the breast crease. But there is no way, if you are submuscular, you can repair the muscle, which I think is incredibly important for best result; otherwise you can get a pec motion deformity, and repair of the pec brings the breast back together and makes it look neater and less flat and spread out after the implant is removed. Hell yes it can. And will not need lengthening, in my opinion. However, especially with removal, these incisions will often tether slightly and must be massaged starting at three week to ensure that they heal smoothly.

After radiation, the skin can be more fibrotic, and there are higher risks of mastectomy flap necrosis and healing complications when compared to a non-radiated breast. Even more importantly, the risk of capsular contracture goes up, especially if the reconstruction is over the muscle. The use of ADM and under the muscle is protective against capsular contracture, so I would still go under the muscle as the standard of care.
