Yes, I usually can and do perform muscle repair after an en bloc procedure. I think it gives a better long term result with less chance of having an animation deformity. Make sure you have surgery with someone familiar with both en bloc and muscle repair. Good luck.

In my experience, if properly released (which sounds like it was given your nice outcome), it shouldn't go back to how tight it was.  It will either be completely resolved or significantly improved. In some cases, a residual amount of memory of that fold comes back after implant removal.  Will need an exam to really know that.

Implant removal and lift is the most common surgery we perform and can be safely perform with great outcomes.  It allows for one surgery and one recovery to get the look you desire.  Select an experienced surgeon in that procedure and good luck.

As you stated, besides implants the only other option to enhance your breast volume would have been fat grafting. However you do not have enough donor fat to get the result you want.  Smaller implants are probably your only option.

Both options given to you are good.My preference would be to exchange both your implants to different sizes and place Galaflex. I found it easier to use, consistent and a cheaper option for lower pole support.  Good luck.

Sorry you are unhappy about the results.  As far as the capsule: a mature capsule takes about 8-10 weeks to form. Most likely early on, you don't have a capsule to be removed.  Good luck.

A breast lift is a great option for you. It would re-position your breasts and nipple into a higher and more youthful position.  A lift alone doesn't involve removing breast tissue; just skin.  Usually that doesn't significantly change your cup size.  Get a consult and learn your options.  I think it would give you a great result.

Usually an increase in 200 cc makes a noticeable difference in your cup size.  Also, 800 cc is the largest silicone implant currently approved in the US.  If you want to go larger, you would need to switch over to saline implants.

Great question!  While it is not "necessary" to repair the muscle, I usually recommend doing so.  After implant removal without muscle repair you have a higher chance of developing an animation deformity which most women do not like.  I usually recommend repair.

I think either timeline should work well. Its best to wait to allow the swelling from your surgery to subside before proceeding with another surgery.  The fat should help give you better shape and help the nipple concavity you have now. Good luck.

You are going to get a lot of different opinions on this question. If you are unsure, then there is no harm in waiting 6 months to see how your skin does before deciding on a lift. You nipples are in good position. Obviously you will have a deflated look to your breasts after your implant is removed but if you skin retracts, a lift may not be necessary.

It is hard to give advice without an exam but what you describe sounds like an animation deformity. That doesn't completely make sense if your implants were placed on top of the muscle.  I would discuss with your surgeon to make sure that the muscle wasn't partially cut or something else not happening. Usually I would advise muscle repair which solves the problem but your case is unique and I would need more information and an exam.

It is unclear of what options were presented to you prior to your surgery and how you and your surgeon made your original decision to just place implants.  However, based on your pre-operative photos it appears you've always needed a lift.  Getting implants alone have helped add volume and fill some of the skin envelope but a lift would fix the rest if that is what you want.

Your situation is rather common issue with women after pregnancy/breast feeding.  You have a deflation of the breast tissue with upper pole volume loss.  You also have some excess skin and glandular ptosis. Which surgery you decide depends on many factors and is a rather long consultation I have daily with my patients.  To fix the volume and shape of your breast, a lift and implant is ideal.  If you are adamant that you don't want the incisions, then you need a rather large implant to fill...

Fat grafting can be done in conjunction with implant removal (even if en bloc) or performed later at a separate surgery. As long as you have the minimal amount of donor fat to use, then it is a good option to give you some shape back after implant removal.
