It does appear the implants have at least on one side bottomed out again. This is not an unusual problem with implants this large or tissue which has lost its elasticity. It is very possible that your tissues have lost a great deal of their elasticity with your significant weight loss. In that scenario larger implants will tend to bottom out more because those larger implants weigh more. Activity level can also play a role. It appears that you would benefit from waiting for a total of about three months until the scar tissue has had a chance to mature. Once everything has softened then you can consider a revision of your lift/scars with or without replacing of implants.  Probably none of that sounds like what you wanted to hear but  from what I can tell in the pictures you should be able to have a good outcome from this.

I agree, you appear to have implant malposition with vertical excess.  You would benefit from some internal modification of the pocket with cautery, sutures, and/or mesh.  Also, I would recommend removal of excess skin (revision of the lift) to shorten the distance from the nipple to the fold at the lower border of each breast.  I would recommend that you discuss these issues with your surgeon.

In my opinion you would have a better result with a vertical mastopexy (lollipop breast lift).  A doughnut lift would not give you a favorable breast shape.  You may consider an implant but that is a personal choice.  Certainly you don't need a large implant.  Best of luck!  You should be able to achieve an excellent result.

Leaving your Steri-Strips in place for another week or 2 should not be a problem.  Sometimes patients will have a skin reaction to the adhesive after prolonged use but if that is not a problem then go ahead and leave them there.

Probably your scars have reached maximal improvement at this time.  Although you may see some minor improvement, I would not expect much.  You should probably consider seeing a plastic surgeon about a scar revision.  I would recommend that you ask about optimizing scar management to get the best possible results this time.  Options include topical silicone (liquid or solid), Kenalog injections, laser treatments, and wound taping.  Best of luck!

What I seen in your photographs is not unusual after a lift and augmentation.  Absorbable sutures are usually used because they give you a better scar.  Alternatives would be placing sutures in your skin or staples in your skin which would have to be removed later.  These almost always cause some degree of crosshatching of the scar at the skin level.  Your surgeon may vary the type of suture used but I would still recommend an absorbable suture.  These don't look too serious.  I would fol...

You appear to be a better candidate for a lift (with or without implants) than fat grafting. Scars should heal nicely after a mastopexy on you.  Although it is hard to guarantee scar resuts, it could be that looking at postop photos from your plastic surgeon of choice would make you feel more comfortable.  Scar management postop should also be a topic of discussion.  Best of luck!

Certainly you can downsize and I would recommend dropping at least 100mL in volume.  Reducing the pocket to fit the smaller implant can be accomplished with a cautery technique, a suture technique, or a support material technique (such as ADM or mesh).  It might also be worthwhile considering raising the inframammary fold as well with these same techniques.  As there are many qualified surgeons in your area, I would advise a second or third consultation so that you are comfortable.  Best of ...

Changes can occur in the post operative period.  Smooth vs. textured implants, weak vs. strong tissue, dissection plane, tissue reapproximation, post-op support, post-op activity, implant size, and other things all play a role in final implant position.  I would recommend you share your concerns with your surgeon and see what can be done.  I wish you the best of luck.

The style 107 implants are in fact a little bit less prone to rippling.  This will probably not make a tremendous difference in your surgery.  You should of course discuss this at the time of your consultation with your plastic surgeon.  I hope that helps.

An areolar reduction would not be sufficient for you and would yield a bad result.  I would recommend you consider a breast lift.  You could also consider a breast reduction if you think your breasts are too large.  It appears the you have some breast asymmetry.  Your best results may come with a lift on one side and a small reduction on the other side to make you more even and lifted on both sides.  A few minutes with a board certified plastic surgeon should make you feel much more comfort...

It is difficult to tell from your photograph.  In general, a hematoma will result in one breast which seems larger than the other side and feels more painful than the other side.  My guess is that you simply have postoperative bruising and it will resolve on its own.  If your breasts feel the same size and have about the same amount of discomfort then you probably do not have a hematoma.
