I do recommend following up with your surgeon to examine the site. With that said, some partial dehiscence/wound breakdown is not uncommon and usually heals without intervention. From time to time I have used advanced wound dressings (collagens, silver based dressings) to expedite healing. Ask your Dr.

Bellafill is one of my favorite fillers for nonsurgical augmentation of the midface, nose and chin. I absolutely do recommend trying a hyaluronic acid based filler first (ie Juvederm) like you did, as it’s completely dissolvable, while Bellafill is not. I recommend a conservative approach—1-2 syringes Max per session to avoid overcorrection.

Partial wound dehiscence is not that uncommon after body contouring surgery. Wound care is based on amount of drainage and wound cultures. Culture-specific antibiotics and protecting the surrounding skinflint moisture are key. Aside from packing, I find negative pressure wound therapy (KCI-wound VAC) may expedite healing. The good news is that with time, this will heal, so hang in there.

You are very early in your recovery period. You can always go bigger, but I would try these out before making any decisions. Implants drop/descend 3-6 months postop (I have found it takes a bit longer in petite, athletic patients like you). I believe revisions should only be done after 6 months to ensure you make the right decision.

Scars are mature 12-18 months after they’re made. Until then (or in the immediate postop period), keeping them out of the sun, avoiding frictional forces, and barring any wound healing complications, will ensure an optimal appearance. After that, scars with a pink hue respond well to a broadband light laser (bbl). Thick or raised scars respond to a few sessions of injection with 5-fluorouracil +/- Kenalog (if your skin color permits).

Your ultimate cup size will depend on the additive effects of implant + your breast tissue + where you get fitted for a bra. Cup size is very subjective. We aim for a final appearance rather than a cup size to ensure your aesthetic goals are met. Do try on some sizers in your Surgeon’s office, and do send your surgeon some representative pictures of your desired outcome (a wishlist). Best of luck.

Facial contouring and final appearance is a matter of adding and subtracting volume in the right places and in the right proportions. I agree with the other surgeons in that an exam is needed to tailor the right approach to your desired goals. I combine buccal fat reduction with masseteric Botox to decrease lower facial volume with fillers/grafted fat to augment other areas. The result is a youthful facial shape (a triangle with the point facing down) in the correct proportions.

Overresection, paired with fat loss that occurs with normal facial aging, can lead to a gaunt/too thin appearance. I perform buccal fat reduction (not complete excision) to lessen the likelihood of this. A conservative approach is best (much like treatment for lower eyelid fat), as this procedure yields permanent results.

Great question. I compare buccal Fat reduction (I don’t remove the whole pad, as I feel there is no need) adds similar effects to those you’d appreciate with facial contouring makeup. Yes. The HA component of Bellafill lasts up to 5 years; the PMMA microspheres last much longer!

I prefer Juvederm UP XC for lips. Belotero is a less viscous alternative but I do feel it is better suited for tear troughs (under the eyes) and other areas with thinner skin (vertical lip lines, the ones lipstick bleeds into). With Belotero, I believe you will have an even more subtle result. Perhaps try a full syringe of Juvederm next time? Best of luck.

Pre-rupturing an implant may help address size and degree of ptosis (whether or not a lift is needed). I also recommmend rupturing the contralateral side if one side is ruptured to allow some “recoil” of the breast tissue and a more symmetric result. I often change planes (above to below) when I exchange implants to deal with the expanded pocket if a lift is not also performed.
