Thank you for your question. It is always best to try and get as close to your goal weight as possible prior to surgery. Breast size will often change with weight loss or weight gain. Your surgical results will be much more predictable once your weight is stable.

You can certainly get a breast lift which will reduce the size of your areolas.  Occasionally a small amount of the pigmented areola tissue that would normally be removed has to be left to avoid excess tension on the skin.  That is best determined at the time of surgery.  It can be removed later if it is an issue.  I would recommend doing the lift first and then coming back at a second operation to add implants if you would like more volume.

Based on your photos you are a good candidate for a breast reduction.  Other factors are considered and discussed during your consultation.  Insurance requirements vary from policy to policy.  It would be best to find out from your insurance provider what the requirements for coverage are.  If it is not covered by your insurance you can still do the procedure but it would be considered cosmetic.

It would be best to have a consultation with a plastic surgeon to determine if a breast lift is a good option for you.  If the position of your nipple drops below the crease of your breast then a breast lift is typically needed.  If your surgeon feels that a breast lift is best, part of that operation is to reduce the size of your areola.  If it is felt a breast lift is not necessary an areola reduction can be done instead.

Liposuction will be very effective at removing the excess fat in the abdominal region.  However, liposuction does not tighten skin.  You do not appear to have much excess skin in the upper abdomen and so a mini-tummy tuck to address the skin on the lower abdomen may be a good option if some loose skin is a concern.  Meet with a board-certified plastic surgeon to better understand all of your options.

In my practice I would not have concerns about proceeding with a breast reduction.  It is best to notify your surgeon about your concerns to avoid any confusion about his/her preference regarding medication avoidance prior to surgery.

It is still too early to expect the implants to drop into their final position at 6 weeks.  It will take several months to see the final result.  By 6 months it is unlikely the implant position will change.  Discuss your concerns with your plastic surgeon and give it some time.

You appear to be recovering well.  The skin will contract some after implant removal.  It is best to wait about 6 months to see how the shape ends up.  Currently the nipple position appears slightly low (moderate) but it is difficult to tell for sure.  If after 6 months you would like to improve the shape, a breast lift can be done.
