That’s a clear high grade diastasis. That won’t improve with exercise or PT at this point. It would not be amenable to a laparoscopic or posterior repair. With that degree of weakness, I would usually suggest mesh reinforcement of the repair, as conventional muscle tightening we do on normal degrees of diastasis would probably fail over time time.

Actually just finished a similar case using Durasorb just today. Durasorb is made out of material (polyester filaments) with decades of clinical use, just in a slightly different form. It has a very small footprint compared to traditional mesh sheets and wouldn't be palpable in this context. It was actually designed in response to the tendency of traditional sutures to cheese wire, with the idea the wider surface area distributes point tension better.

I would not call that a tuberous breast deformity. It really doesn't have strong features of the classic term.

I would suggest you would need a lift, and I would not use high profile. In fact, id suggest using low or low-plus profile devices (which do very well with lifts) as you already have a lot of breast tissue and it's easy to overshoot the mark from tasteful to tacky volume wise.

You could go sub fascial as you have thick tissue, but they will sit a bit lower then a dial plane if...

As you have a visible double-bubble (crease of your old inframammary fold) showing on the left, they absolutely lowered your fold to accommodate a larger implant. I'm not a fan of that maneuver except in some severe congenital deformity cases as they can settle MUCH lower then intended as here. Sometimes that's inevitable btw if you have stretchier tissue and wasn't a technical error during the procedure. That probably will require mesh (Galaflex or Durasorb) to hold the position you're...

Liposuction alone would not get you a very satisfying result because of the amount of skin excess you have and the visible laxity of your abdominal wall underneath. For best result a tummy tuck would seem obligatory. You do have some back and flank fullness that would do well with liposuction tho.

If your priority is the BBL, that's actually better done as a standalone thing forst rather then trying to do it at the same time as your tummy tuck, so you could sequence it like that. I would...

I get what your consulting surgeon was saying. You clearly couldn't get the skin down to to do a traditional abdominoplasty while a mini is going to leave a lot of the attenuated akin at your belly button. You otherwise do kind of look like a mini abdominoplasty candidate (with muscle plication, in my hands)

The compromise would be to have some type of lower abdomen vertical scar component. I'm guessing it would be about 2-3" . That can look good contour wise if you will accept the scar....

Anyone suggesting liposuction alone will.give you a good result has rocks in their head. You have very obvious significant muscle laxity of your abdominal wall, to the point it's a legit question as to whether mesh reinforcement may be indicated rather then just repairing the diastasis with sutures only.

If you are wanting a BBL that is probably better done as a standalone before to realize more fatty tissue to graft, but just be realistic that an abdominoplasty would be needed later for...

A Benelli "doughnut" mastopexy is almost NEVER the right answer. Very unpredictable results and bad scars are the norm. You pictures suggest a vertical scar approach might work, but it could require a short T or J shape for skin excess (always hard to tell for smaller cases). I think short T scars look better if you weren't getting an implant to take some of the slack out

It almost certainly was the obesity rather then the methadone that resulted in the gynecomastia (very common in male bariatric patients)

Usually this would be done a plastic surgeon whom specializes in weight loss patients. They should be able to address both areas which are common procedures for us that do a lot of weight loss patients. A significant male or female FUPA can require a second dedicated surgery sometimes if there's a lot of bulk and skin as it can require a T shaped scar

Not a fan of that "hemostatic neck" facelift approach you had personally as it's not clear they look better over time and it's a very tedious apply method. (Not much juice for the proverbial squeeze)

Would give that band like area some time and gentle massage. That kind of thing can soften away remarkably over time. Otherwise, I think that will end up looking pretty nice as it settles in

My hunch is that's a foreign body response. As it's so demarcated my guess would be it might be from some fillers along the orbital rim. Some people have experienced this kind of thing after COVID infection/vaccination

An MRI can probably give you an idea of that, or you could try a hyaluronidase injection (an enzyme that dissolves HA fillers) to one side and assess the response

You have a mild degree of "levator ptosis". The surgery to fix that is called a levator advancement where they shorten and reattach some stretch fibers in your upper lid. Not super complicated, but you want someone who does that particular surgery frequently like an oculoplastic surgeon or plastic surgeon whom does a lot of this specific lid surgery (not just cosmetic blepharoplasty)

You would probably need an abdominoplasty rather then a mini abdominoplasty there. The only patients that do well with mini’s typically look pretty taunt/flat prior. I suspect you’d have more loose skin to deal with (even though your picture looks ok)

Routine use of mesh in primary breast augmentation would be irregular unless perhaps one was doing a subglandular placement (rather then partially under the muscle). It would normally be reserved for patients with very poor tissue quality, combined mastopexy-augmentation, or revision breast surgery. With a moderate sized implant as you describe I can’t imagine it as to be necessary in this context
