If the firm areas are still present after a year, I would recommend obtaining imaging for further evaluation. Fat necrosis has specific findings on imaging. This may also help provide information regarding the size and consistency of firm area. Based upon this, further intervention can be discussed. I would follow up with your surgeon to discuss options at this point.

Would Ellacor be a viable option to help lift the philtrum?

Micro-coring allows for removal of skin in the upper lip. This would allow for lifting to occur. As this is the only non-surgical way of skin removal, this is the best option for lifting the philtrum while avoiding an incision. I would recommend discussing this with your Plastic Surgeon. The concern relates to wanting to avoid any complications including injury of the implant or infection. After an examination, your surgeon can answer the probability of this occurring and if other options would be more beneficial.

I would recommend allowing time to heal. This will allow for all of the swelling to resolve. You would have a better idea of what size you would like to be with an implant exchange. Six months would allow for this to occur.

Though there is a difference between the high profile and moderate profile, I would recommend allowing longer time to heal to make sure you are happy with your decision. Early postoperative period has swelling and healing that is occurring. I do not see an urgency to perform an implant exchange at 1 week. Discuss this with your surgeon and obtain a second opinion if you are not confident.

Galaflex mesh is used to provide internal support for both soft tissue and for implants. The larger the implant, the more support needed long term. Based upon your photos and desire to go larger, use of Galaflex mesh seems appropriate. Every plastic surgeon will have their preferred method. Good luck with your revision surgery.

Hi, the most important issue to address is the leaking silicone implants. If this has leaked into the breast tissue, it is more challenging to completely remove. With above the muscle implants, there is an increased of healing issues with a mastopexy. I would discuss with your plastic surgeon the reasons you are requesting this procedure. The areolar incision is small and subsequent surgeries risk puckering in this area or sensation loss. Being under the muscle would make this challenging to perform any significant capsule removal.

Draining the seroma can safely be performed under ultrasound guidance. Though an infection is a risk, when performed with sterile technique, needle aspiration can be easily and safely performed under local anesthesia. The swelling may be a liquified fat cyst from the transfer that upon drainage, does not return. I would recommend following up with a board certified plastic surgeon for imaging and further management options as this is not likely to resolve without intervention.

A few different issues may be contributing to your current presentation. An ultrasound is a reasonable place to start, not only to evaluate for a seroma (fluid collection) but to also evaluate the abdominal wall muscles. Laxity in the abdominal wall occurs due to weight fluctuations, pregnancy and as we age. After 6 months, the results will not likely change going forward. Ultimately, a revision surgery to make the right side slightly smaller would depend on how much the size difference bothers you. The risks of a second surgery would include worsening of the numbness that you have. I would discuss your concerns with your plastic surgeon and determine a plan moving forward. I would follow the recommendations based upon your mammogram findings. If this was normal and no further imaging was currently recommended, an ultrasound specifically looking at breast tissue may not be needed.

An ultrasound to evaluate the implant for rupture, presence of seroma or capsular thickening can be performed by your plastic surgeon. This would not diagnose cancer but could help with the decision making process moving forward.
