FTM “top surgery”/chest masculinization (female to male surgery) (which does not quite count as a mastectomy, in that 10-20% of breast tissue is left behind) can be done via a technique of leaving the nipple attached (rather than grafting it), so that some sensation is retained. It is a very safe and highly effective surgery that has been continued to be refined over the years, and has treated trans-men from all over the country (as well as few from overseas).

What are the variants of “top surgery”/chest masculinization?

In relation to the nipple-areolar complex, there are in principle three versions of chest masculinization (female to male surgery)

1. With free plasty of the nipple-areolar complex. Assumes the removal of the mammary gland from horizontal (or semilunar) incisions.

The nipple-areolar complex is isolated from the removed mammary gland, after which it is transplanted to a new site (acceptable from the cosmetic point of view).

This type of operation is used for 3 or more sizes of the breast, as well as with a smaller size of the breast, but with a pronounced omission of the mammary glands.

2. With displaced nipple-areolar complex. The mammary gland is removed from the horizontal or semilunar incision, while the nipple-areolar complex remains on the epidermis cutaneous leg (through which the blood supply is carried out). This type of operation is used at 3-4 sizes of the mammary gland, relatively unexpressed mammary gland lowering.

3. Without moving the areola. The mammary gland is removed from the paraareolar incision, while the nipple-areolar complex remains in the same place and excess skin is excised around the areola. This type of operation is used for 1-2 sizes of mammary glands and absence of mastoptosis.
