Can my forehead birthmark be removed? I have asked around and all have told me no.
Surgical excision
Jul 11, 2019
To remove a raised dark birthmark such as yours would require a surgical approach. This means that there's a tradeoff with scarring. In my experience, lasers are good at converting one type of patch into another. Albeit, they can improve contour, I'm not sure they are great at eliminating a flatter patch-like appearance. On the other hand, a surgical excision for a birthmark like this is not always linear, but the face tends to scar well as long as no distortion is introduced.Dr. Panossian's...
Do you have any advice on non-surgical rhinoplasty (fillers) while I save up for a revision?
Nonsurgical rhinoplasty in lieu of revision
Jun 18, 2019
Non-surgical rhinoplasty is an excellent way to correct small nasal irregularities and to simulate what correct aesthetic nasal proportions should be for your nose. There are definitely pitfalls and dangers associated with liquid rhinoplasty, but done correctly, they are very safe and effective in temporizing the appearance of a less-than-ideal nose.Read about Dr. Panossian's rhinoplasty technique.
What can I do to get a softer, more rounded nose tip? Would de-projection be visible from a front-f
softer nasal tip
Jun 18, 2019
Thanks for your question. Your tip and hump can certainly be improved to more or less the degree you demonstrate in the morphed photo. Deprojecting a tip can make the nose look a bit wider on frontal view as well as relieve a tension tip. However, there's more to it from a technical standpoint. To get into the technicalities of the procedure is not necessarily of value to patients but moreso for plastic surgeons in training. Getting you ultimately to your desired goal requires a series of ...
Nostrils to narrow after rhinoplasty. Can these be widened?
Widening nostrils
Jun 9, 2019
Nostril widening is possible using composite ear cartilage grafts. However, maintaining proportionality with the rest of your nose is key. You must understand that changing the proportions in the way in which you desire may unmask a different problem. This is a visual effect. A properly done rhinoplasty takes into account the correct aesthetic proportionality of the nose. Changing a single element is not always a good idea without a good comprehensive plan and a list of maneuvers needed to ...
What surgery options to get this type of nose?
Rhinoplasty goals
Jun 9, 2019
To a certain degree, a person's nose can be made narrower and more refined. The limitations lie not necessarily in the surgeon's ability, but a combination of his/her judgment and in the tolerance of the patient's tissues to achieve those goals. Therefore, having realistic expectations is key. Computer morphing and digital photography are great tools, but they may give you a false sense of what is achievable. The problems I see with your nose is that the skin envelope is thick and small and...
How long after liposuction can I get rhinoplasty?
Liposuction and rhinoplasty
May 31, 2019
The answer to your question really depends on the extent of the liposuction. Small areas of liposuction can be combined with rhinoplasty during the same surgery. However, if it's large volume liposuction, then you may want to separate the procedures. I would give it at least 6 weeks between surgeries so you can recover fully before undergoing general anesthesia again. There are no set rules for this, but I would give yourself a bit of a break to heal. Also, I would recommend doing ...
Revision male rhinoplasty - how many millimeters can you add back?
Revision rhinoplasty for men
May 29, 2019
Thanks for your question. It's a bit difficult to thoroughly answer your question without photos or a consultation, but the short answer is yes, you can add length to your nasal tip. However, everyone's capacity may be different. The more important question is what do the proportions of your nose look like? In other words, the length and width of your nose matters in these decisions. Rhinoplasty or revision rhinoplasty is about restoring or improving nasal proportions so that your tip...
1 year post-op. Does rhinoplasty swelling cause the septum to look longer?
Long nose
May 29, 2019
Thanks for your question. The question you are asking is related to nasal proportions. Large or long noses can still look long or even longer when the dorsum is reduced in height and the nasal bones are narrowed. Without addressing the nasal base, this look can persist. In looking at the one photo you have provided, your columella appears to be hanging and your tip could be underrotated. This could give you the look you are seeing. Although you will need a full in-office consultation, I ...
What is this depression along my nasal bridge and is revision necessary?
wide hump
May 14, 2019
It does appear that you do indeed have secondary issues related to your dorsal hump. Unfortunately, a revision is what is needed. There are many ways to approach this to ensure better alignment, width, and contour. Additional photos, or in-office consultation, would be required to narrow down the technical plan in order to get you to your goal. This can be achieved through closed rhinoplasty.

3 weeks post-op. Is this painful lump under my breast normal after a lollipop with lift?
Painful lump after breast reduction
Apr 18, 2019
Thank you for your question. The lump itself occurs at the furthest corners of an incision and is called a "dog ear". Removing it sometimes requires a small horizontal incision along the breast fold. However, many times after a lollipop incision this lump may be left behind to flatten on its own. It does usually go away in this setting. Sometimes, if it is painful, there may be a stitch trying to spit. Usually, not much to worry about but, of course, check with your plastic surgeon and go w...
Is this nipple necrosis, or bruising?
Bruised nipple
Apr 12, 2019
You may just have some congestion and bruising of the nipple, although it's difficult to say with certainty. I would follow your surgeon's advice on how to proceed. Usually, if this were nipple necrosis, you would see a more clearcut scab or eschar where the circulation was poor. --Dr. P.Read about Dr. Panossian's approach to breast reduction.
Can I have a BBL, TT, lipo and rhinoplasty all at once?
Split up your surgery
Apr 12, 2019
When combining plastic surgery procedures, it would have to be first and foremost safe, and secondly, considerations on positioning and recovery must be taken into account. Personally, I would separate out the procedures such that the recovery of one procedure is not impacted by the recovery in another area. I am very careful with my BBL patients because I do not want shifting of the fat and to give the injected fat as much opportunity to take as possible. Maneuverability may be affected i...
Follow-up question: 9 months post-op. Is my nose still upturned?
Upturned nose
Apr 12, 2019
There are many reasons for why a tip would be upturned after rhinoplasty. One reason is that there was perhaps an over-resection of some of the cartilage components of the nose allowing the tip to rotate upwards. It is a difficult maneuver to perform correctly and to avoid secondary breathing issues. Another reason is that this may be the preference or "taste" of your surgeon. Rhinoplasty results need to be discussed during consultation so that you are aware of what your surgeon's taste is a...
How can I fix my nostrils show from the front view?
Fixing excessive nostril show
Apr 10, 2019
You may get differing opinions on this, but the reason for nostril show in your case is that your lower lateral cartilages are either weak or deficient causing a notched appearance. This is most commonly seen after rhinoplasty when there has been an excessive cephalic trim of the lower lateral cartilages. In your case, this is compounded by the fact that your alar base is lower than the columellar base. This will give someone the appearance of flared nostrils. In this setting, a c...
Is this nose job possible for me? Rhinoplasty
Marilyn's Nose
Apr 3, 2019
Thanks for the interesting post. Marilyn Monroe had a very classic, natural look...one that we have moved away from with improvements in rhinoplasty and refinements on the concept of nasal aesthetics. With that said, nowadays we have more control (as plastic surgeons) on the final appearance of the nose. Your current nasal tip does have a bulbosity and does overproject slightly, giving you the appearance of having a big nose. You would need to have the tip de-projected slightly, but your...
