Though its hard to tell the exact position of the lower breast tissue and nipple from the single view photo and with the arms raised, my bet is that you have a moderate degree of nipple ptosis and therefore would need a lift with augmentation. Based on the one photo, it appears that you have moderate ptosis, some enlargement of the areola and some constriction of the lower medial portion of the breast. Mastoepxy augmentations can be challenging operations, especially in post partum women who have often loss of eleastic recoil in the lower pole of the breast. The other thing to consider is your native “breast footprint.” If you had lower positioned breast from the beginning, that may limit the amount of upward movement that an implant can acheive. I do not detect any true bottoming out, as that term is used to describe a situation where the implants fall below the lower breast fold. From the side view, your nipples are in correct position. If you are dissatisfied, I would speak with your surgeon. The difference between intial post-op and now is that the tissues have relaxed to accommodate the implant and the swelling has subsided. This is the natural progression that we expect.

Although there are still some changes happening at 6 months, I agree that the incision is a little high and you have a dog ear on the right side. The dog ear can typically be easily revised in the clinic setting. Revision would have to be performed to lower the entire incision. It would depend on the laxity of your skin whether that is feasible.

Some people do react poorly to certain suture material resulting in the suture “spitting” or being expelled from the tissues. Typically, and pus or reaction associated with this dissipates once the suture is no longer there. I would first talk to your plastic surgeon.

A maximum BMI is surgeon preference. Of course, we all want the patient to be closest to and stable at ideal BMI but I realize that is easier said than done for a lot of people. Not all BMI’s area equal and it would depend for me on how your weight is carried and the presence or absence of other health risks. My number one priority is the safety of my patients.

There is absolutely still potential for remaining swelling 3 months after surgery but it should be starting to go away. Important factors to help control swelling are regular use of compression garment, healthy diet (with lean protein, low sodium and low carbohydrates and sugars), and maintaining a consistent weight and body composition. I would recommend talking to your surgeon also to make sure you don’t have a fluid collection or seroma that can mimic swelling but require treatment.

If you have fluid between the layers of your abdomen (seroma) you need to see your surgeon and get the fluid removed. Your body can only handle a small amount of fluid before it begins to wall it off into a permanent seroma. If you simply have fluid build up within the tissues (swelling) then drink water, avoid salt, rest and the body will take care of it.

You can try a scar therapy such as silicone sheeting or embrace to lighten and flatten the scar. It is not uncommon to adjust the most lateral ends of the incision (know as “dog ears”) to avoid a long scar. Usually that is done in the office w ith local.

Yes, occasionally. The healing process can take several months. If the pain is getting worse rather than improving, you should see your surgeon to make sure you are not having a complication.

If you had a tummy tuck alone without other intrabdominal surgery, the surgeon would not have been anywhere near your bladder or nerves to the bladder. My suspicion is that whatever you are experiencing is not related to a tummy tuck from 6 years ago.
