I would need to see your pre -operative photos to be sure, but that extra skin may be where your natural breast originally started off. The natural crease of the breast that has been distorted by the large implant. This is a complex problem, with different options for solution. I would need to exam you and discuss all of the options. Best to find a plastic surgeon comfortable with breast augmentation revisions and complex breasts surgeries as unfortunately you problem is not an easy fix.

Weight gain after liposuction of stomach and thighs with fat grafting of hips is most likely due to swelling and fluid retention after surgery. lymphatic massage may be helpful. It take several weeks to months for all the swelling to resolve. Eat a healthy diet and simple exercise is fine.

Your photo does show swelling and irregularities in contour after your abdominal liposuction. You did not say what kind of liposuction you had ( vaser, Pal, smartlipo) . At this point the best way to reduce this is with the proper compression garments and lymphatic massages. In my office, we provide our patients with a list of recommended therapist and garment providers. Look for massage therapists that work with post liposuction patients. we can give you our list but we are in New Jersey....

I would suggest you lose more weight prior to having a tummy tuck to get your best results. At a BMI of 47, there is an significant increased risk of complications. Bariatric surgery for weight loss may be an option for you, combined with nutritional counseling and your exercise routine. There is a procedure known as a panniculectomy, which removes the lower fold only. It is not a cosmetic procedure but mat help. Consult with a board certified plastic surgeon.

The outer thigh liposuction has removed the fat but has left you very straight, judging from your photos. Some people like this look but obviously you do not. It appears that your best option would be fat grafting that area to give you more curve.  from you photo you may have some fat at the lower back/ upper buttock junction that can be harvested and placed to restore your curve. I suggest a consultation with a board certified plastic surgeon who is comfortable with these procedures.

Healing after a tummy tuck or any surgery is a long process. I use a scar control program in my practice to try to prevent keloids, raised scars and dark scars. We start the program a few weeks after surgery and continue for weeks until the scars look good. I can't be certain, but from your photo it appears that your scar is getting raised. It may be a reaction to a suture or scar thickening. I would return to your doctor for an examination and advice.

Combined procedures are common and beneficial because only one recovery is needed. You may have a slightly longer recovery however. I have found that most patient can go on vacation and walk or ride bikes(gently) around a month after surgery. I typically use a general surgeon In the patient's insurance plan to repair the hernia especially if mesh  is needed.
