There are several treatments available for keloids. Without an in-person exam, it is difficult to give advice for your exact problem but it is possible that an injection of a steroid into the scar may soften and flatten the keloid. Sometimes it takes multiple infections over a few months time.
The steroid does not work for all people and then surgery could be considered.
Think hard before getting another piercing. You may have the same problem even if you don’t get another infection.

It is difficult to answer your question from just one photo. I would like to see a side view and have some measurements.
With that said, it is likely that your nipple is in the correct location but that you have some “bottoming out” with the distance beneath the nipple to the chest wall being stretched out a bit. It may be possible to improve that appearance by removing some more skin at the bottom.

I would not recommend an emergency room visit. The emergency room physician is not likely to be an expert in breast implants. Emergency rooms are expensive and best saved for emergencies.

It is not possible to answer the question of whether your insurance would pay for a reduction for you over the internet. You need to be evaluated by a board certified plastic surgeon for a full evaluation. Each insurance company has different criteria for coverage – I know, not really fair.

I would not say there is something “wrong” with your nipple. It just doesn’t have a perfect shape. If the appearance bothers you, see a plastic surgeon. It may be a pretty easy fix. It is not likely that your insurance would cover the cost. Nevertheless, it may be worth the cost to you if it would make you feel better.

It is interesting that you note your right side is higher and softer than the left. Your photos suggest that you have a capsular contracture on the right making the implant appear higher, or fuller, at the top.
There is nothing that you can do to change this. I recommend you discuss with your surgeon for an in person evaluation to determine if contracture is the actual cause.

I agree with all who have said “No” to a heating pad. Most surgery will lead to a change in sensation of the surrounding skin. This is especially true of liposuction. You can’t feel normally so you can’t tell that the heating pad is too hot. This can result in burns. I have also seen a full thickness burn of the skin when a patient used a heating pad after liposuction. Put your heating pad away please and speak to your surgeon about other methods to…
