Non-incorporated Alloderm occurs when the biologic is not in close contact with surrounding healthy tissues. I have found portions of ADM free floating in the pocket as your describe but that is the the best case scenario. More often than not, non-incorporated ADM is an infection risk.

Many surgeons will avoid liposuction in the central area since that has been associated with wound healing complications. But your concern about stretching is a good one. ADM’s are derived from dermis and by definition have a degree of elasticity. This would most likely require your nipples to be removed, the breast tissue removal performed, the skin re-draped over the new breast, and the nipples placed as skin grafts.Grafted nipples have no sensation and can have pigment changes.
